Vaccines in bold type are required school
immunizations. * May be given as early as 12 months of
age instead. Not needed if the child has already had chickenpox disease. Logan County Health Department also offers other immunizations, including flu,
meningococcal, pneumonia, and adult tetanus. Some foreign travel immunizations, such as hepatitis A and hepatitis B, are also available. For additional information, please

We have two forms that must
be filled out by the legal guardian, the Vaccine
Administration Record and the Clinic Contradiction
Checklist. If these are signed by the legal guardian and notarized, relatives or friends
of the family may bring children in for immunizations.
