Vaccines in bold type are required school immunizations.

* May be given as early as 12 months of age instead.  Not needed if the child has already had chickenpox disease.

We have two forms that must be filled out by the legal guardian, the Vaccine Administration Record and the Clinic Contradiction Checklist.  If these are signed by the legal guardian and notarized, relatives or friends of the family may bring children in for immunizations.
