This program is designed for lower income families of Logan County who are at nutritional risk

Family Case Management
Who is Eligible?
Women,
infants, and children
Program Includes:
Referrals for medical care throughout your pregnancy and delivery
Visiting nurses
W.I.C. supplemental food program for women, infants, and children
Group nutrition classes
Individual education and counseling with a nurse
Referral to community resources after delivery
Family planning counseling after delivery
Prenatal classes (B.E.A.R.R.) for teens
Sick child doctor visits, medicines, and diagnostic testing
Prenatal care for mothers on Public Aid
Spend down including prenatal vitamins, laboratory tests and doctor visits
Breast
feeding support and education

Who Is Eligible?
Family Planning is available to all area residents
Fees are charged on a sliding scale according to income
Services Include:
Pelvic exam, Pap smear, and breast exam
Pregnancy testing
Provision and counseling of birth control methods
Sexually Transmitted Disease (STD) screening and treatment
